Do you accept Medicare or Medicaid HMO’s?

Why do you need a copy of my insurance card and driver’s license?

A copy of your current insurance card is to ensure that the correct information is obtained to file your claim. We also abide by the “Red Flag Rule” which is a state mandated law that requires our office to obtain a valid State/Government issued ID from each patient.

What happens if I arrive at my appointment without my insurance card?

We would need to verify your coverage, deductibles and other patient responsibilities prior to providing service. We appreciate your cooperation on this matter and highly encourage you to bring all required documents at your appointment, otherwise, we may

Is there a fee for a returned check?

We do not accept personal checks.

Should I contact your office if my insurance has changed or if I am no longer insured?

Yes, you will need to contact our billing office with any insurance changes or cancellation of coverage. Failure to inform us of any changes may result in you having to pay in full for your medical services.

I have an FSA/HSA. Do you participate with these programs?

Yes, however, if we are unable to determine the availability of funds in your healthcare savings account we will collect any deductibles, co-insurances and/or co-pays at time of service.

The costs of surgery relate to the surgeon and the assistant surgeon

Are my medical records kept private and confidential?

Your medical file is handled with the utmost respect for your privacy. Our staff is bound by strict confidentiality requirements as a condition of employment regarding your medical records. The staff at Pristine ob-gyn care abides strictly by the HIPPA laws. We will also not release the contents of your medical file without your full confirmed consent.

What are your acceptable means of payment?

Bank check,cash and all major credit cards are acceptable means of payment. Unfortunately at this time, we no longer accept personal checks. For added convenience, we also offer credit services through CARE CREDIT and FUND MY DOCTOR.
