New patient’s requirements

On your first visit please arrive 15-20 minutes prior to your appointment time. We will need you to either fill the requested information at the comfort of your home on your computer or at our office location on our computers. Alternatively, we can also mail you new patient forms to fill and bring with you at your appointment. These forms can be accessed directly from our secure web-based patient portal.  If your forms and other documentation are not completed, you will need to arrive 30 minutes before your appointment time so that you can complete the necessary new patient forms via our electronic medical records. If your insurance requires a referral please bring it with you to your appointment.

Please bring these items with you on your first visit:

  • A valid driver’s license or State ID
  • Insurance card(s)
  • List of medications you take on a regular basis, including over-the-counter
    medications and vitamins.
  • List of surgeries or medical procedures you have had
  • Payment for your co-pay or any non-covered services. For your convenience,
    we take cash, checks, HAS/FSA cards as well as all major credit cards.
  • Please note that new patients can pre-fill their medical history, medications and allergies via our secure web-based patient portal. We can mail your access information to you or in can be picked-u p in person at our office with ID verification. This will greatly expedite your check-in at your first visit. Important Message

We here at Pristine take great pride in patient care. We are updating our current electronic medical record to a new and improved one! This new EMR will allow for a more efficient streamlined process for your care delivery.

As with any new process, there may be some challenges along the way. Please be patient with us as we work through this transition.

We strongly encourage you to register for the new patient portal and complete all your pre-visit check in tasks on your phone at least 30 minutes prior to your visit.

If you had a patient portal prior to December 9th 2024, you will need to re-register for a new portal.
